Thank you for choosing Gulfcoast Brain and Spine Center. If you’re a new patient, we want to help you prepare for your first appointment and make everything as simple and convenient as possible. Below you’ll find important information to ensure your appointment goes smoothly. Are you an existing patient? Our patient portal allows you to easily and securely access your healthcare information.
New Patient Forms

If you are a new patient, you have the option to complete your paperwork online or print it out and bring it with you to your appointment. This way, you can arrive on time and not have to worry about filling out forms when you get here. Simply choose the option that is more convenient for you.

Insurance Information

At Gulfcoast Brain and Spine Center, we understand that insurance plans and coverage can be confusing and difficult to navigate. Our team of dedicated insurance specialists is here to help guide you through the process and ensure that your claim is filed correctly. What to Bring to Your First Appointment

Personal identification. Please bring a form of personal identification, such as a driver’s license.

Insurance card. If you have secondary coverage, make sure to bring that information as well.

Referral forms. Don’t forget to bring any referral authorization forms required by your insurance provider. A few of the most common forms are the Cigna Connect Referral Form, Healthspring Referral Form and the Tricare Humana Military Referral Form.

Recent scans or imaging. Your primary care provider or referring physician can typically provide imaging results. If your scan is over six months old, have your primary care provider repeat the imaging study.

List of medications. Please bring a full list of all the medications you’re currently taking.

Completed new patient forms. If you opted to print and complete your forms by hand, please make sure to bring them with you to your appointment.

What to Expect During Your Appointment

The most skilled care and treatment. At Gulfcoast Brain and Spine Center, our team of neurosurgeons are experts in the care and treatment of conditions affecting the brain, spine, and nerves. We offer personalized surgical and non-surgical treatment options for a full range of neurosurgical conditions. Before recommending surgery, we always evaluate all other possible medical solutions. If surgery is needed, our goal is to provide minimally invasive treatment whenever possible.

Open and honest communication. Dr. Constantine Plakas believes that providing the best possible treatment options starts with understanding a patient’s past medical history and treatments. We partner with referring providers to get a comprehensive understanding of a patient’s condition in order to make informed decisions about their care.

A staff with a heart of service. We treat our patients like they were our own family. We understand that this is a trying time in your life. Every condition involving the brain and spine is critical, so we want you to know that we empathize and will do our best to make you feel at home and in the best hands for care.

Payment Information

We try to offer the most flexible payment options, and work with your insurance company to advocate for the best coverage – coverage that you need. We accept the following:

  • Credit cards. We accept all major credit cards, including VISA, MasterCard, Discover and American Express.
  • Cash and personal checks. Cash and personal checks are accepted, but unfortunately we do not accept business checks.
  • Company flex and health savings plans. Many companies offer medical spending accounts. Make sure to speak with your employer about this option.
